Insurance & Fees

Dr. Hartt is in network with Aetna and Blue Cross Blue Shield PPO.

Fees for Dr. Hartt’s services are calculated in accordance with Current Procedural Terminology. They depend on the complexity of presenting problems and medical decisions, and the length of the appointment.

For clients who have Aetna and Blue Cross Blue Shield PPO health insurance:

You are responsible for the payment of co-pay, co-insurance, or deductible fees at the time of the visit. We will check on your benefits and directly bill your insurance company for the remaining balance.

If your insurance company declines to pay for Dr. Hartt’s services, we will make every effort to resolve the reimbursement issue. However, payment for the services you received is ultimately your responsibility.

For clients who have health insurances that Dr. Hartt is NOT in network with:

At the time of your visit, you will be responsible for full payment according to Dr. Hartt's private fee schedule.

If your health plan includes out of network benefits, you can be reimbursed for Dr. Hartt’s services. Please contact your health insurance company to find out how much you will be reimbursed, if there is an out of network deductible, how to file a claim, and other details.

We will provide the documentation necessary for you to request reimbursement from your insurance company.

Minor Clients:

If the patient is a minor, only a custodial parent or guardian can authorize treatment. The custodial parent or guardian is fully responsible for payment for the services.

Payments can be made with a credit card or cash. We do not accept checks.


Office Policies

Cancellations and Missed Appointments

We respect your time and never "double book" appointments. If you are unable to keep an appointment, please cancel it as soon as possible. There is no charge for an appointment cancelled at least 24 business hours in advance.

We understand that there are urgent situations when you aren't able to cancel your appointment in advance. In this circumstance a fee of $50.00 must be paid before another appointment is scheduled. This fee, which is not a full appointment rate, allows us to share the cost of this unforeseen situation with you, while partially compensating us for the loss of time reserved for your appointment.

Missing 2 consecutive appointments or 3 appointments within 6 months may result in being discharged from Dr. Hartt's practice.

Messages

Messages can be left on the office phone voice mail, and will be returned no later than on the next business day. For various reasons, Dr. Hartt will not communicate with patients via email, hence, email messages should not be sent regarding appointments or other matters relating to patient care.

A Parent Must Accompany A Minor Patient to Each Appointment

An 18 year old who is not living at home is considered an adult. If the patient is 18 years old, living with the parents and attending high school, he/she is considered a minor.

Please inform Dr. Hartt in advance if a person other than a parent/legal guardian will bring a minor to an appointment. Written permission for the person to participate in a psychiatric evaluation and make medical decisions, including treatment with psychotropic medications, must be provided by a parent/legal guardian. Your can obtain the necessary consent form from Dr. Hartt’s office in person, or via fax or email.

Prescriptions and Refills

During your appointments Dr. Hartt will provide prescriptions for enough medication to last until your next visit. For example, if a follow up visit is planned in 3 months, Dr. Hartt will give you either a 30 day supply prescription with 2 refills, or a 90 day prescription with no refills. New regulations permit issuing up to 3 months worth of prescriptions for controlled substances (ADHD medications, stimulants, etc.).

We will be happy to fax your prescriptions to the pharmacy directly from our office immediately after your visit and upon your request.

We encourage you to schedule a follow up appointment right after your visit and before you leave the office. This will allow you to get the next appointment on the date needed, and give you the opportunity to let Dr. Hartt know right away if your prescription isn't sufficient for the time between visits. You should not routinely need additional prescriptions or refills between your visits.

In the event that a prescription/refill is needed for reasons that are out of Dr. Hartt's control, for example a missed or not timely scheduled follow up appointment, loss of the original prescription and the like, a fee of $10.00 will be charged for the each prescription or refill approval. In these cases, it will be necessary to have the pharmacy fax us a request for a refill, call the office to inform us about the medication request, and pay the fee.

If a prescription for a controlled substance (ADHD medications, or stimulants) needs a replacement (most often because it has expired), a fee of $15.00 per prescription will be charged. In these cases, it isn't necessary to call your pharmacy as these prescriptions can not be faxed or called in.
